Office Practice

Hours

The office is open from 8:00 AM to 6:00 PM Monday through Friday. When the office is not open, your call will be answered by our answering service and paged to the physician on call. There is always a physician available to you, 24 hours a day, so please call us if you have any concerns.

Appointments

Appointments are available during office hours. While we do not have walk in hours, we can usually accommodate same-day appointments for urgent problems. If you need to cancel an appointment, please give us 24-hour notice so we can accommodate other patients on the waiting list.

Emergencies and Hospital Care

Our physicians admit to Beth Israel Deaconess Medical Center. If you think you might need to go to the hospital for an urgent matter, please call us first if time allows. A physician is available 24 hours a day, and can facilitate your treatment there. If the matter is life threatening, go straight to the hospital and ask the hospital to notify us.

Care Teams

In order to provide continuity of care, we've established physician/nurse practitioner and physician/physician assistant teams. When you come in for an appointment, we'll try to schedule you with a member of your care team. This ensures that you'll be seen by a provider who knows you and your healthcare needs. However, you may also schedule an appointment with any of our providers and be assured that you'll receive quality care.

Triage Services

A triage nurse practitioner or physician's assistant is available by phone Monday through Friday, 9:00 AM to 4:00 PM, to address your questions and concerns. She can discuss your symptoms, medications and/or lab results. She works closely and communicates with your care team.

Prescription Refills

Routine prescriptions can be renewed by phone and called into your pharmacy within 48 hours.

Referrals

Most insurances require referral authorization for appointments outside Urban Medical. Once you have made your outside appointment, call our Referral Line at 617.522.6876 Extension 12 with the following information:

  • the name and address of the physician you're going to see
  • the date of the appointment
  • the reason for the visit
  • your primary care physician at Urban Medical or, if not your PCP, the name of the physician who referred you to the specialist
  • your insurer's name
  • your full name and date of birth.

You may leave this information on her voice mail, but please leave a number where you can be reached.

Lab Hours

We can assist you with blood work or lab tests your physician may require during your visit. If you are coming in for a lab test only, please call by 1:00 PM the day before to give us your name, phone number, the name of the provider who ordered the test, and what test you need. The hours for lab-only visits are Monday through Friday 8:30 AM - 5:30 PM. If you know you are going to have a fasting cholesterol or blood sugar test, please call to ask for instructions.

Medical Records

Your records are confidential and cannot be released to another person without your explicit permission. We are required to have you fill out and sign a medical release form. It takes approximately two weeks for us to fulfill medical record requests. There may be a charge for this service.

When you first visit Urban Medical, you may want us to have a copy of your records from your previous medical providers. We can give you a form to complete for this purpose.

Billing and Insurance

We accept almost every insurance plan; please call to ask about yours. Most managed care plans require you to choose a Primary Care Physician or PCP. You must notify your insurance company that you have chosen one of our physicians as your PCP or your insurance will deny your bills and you will be responsible for payment.

Many insurance plans have a deductible or pay only a percent of the allowable fee. Deductibles and co-pays are the patient's responsibility and are collected at the time of the visit. We accept VISA and MasterCard for charges of $10 or more.

We have a sliding scale fee for patients without insurance. Ask the billing office to set up a plan for you. In cases of financial hardship, we can work out a payment plan if you contact our billing department as soon as possible.
