Community-Based Practice

For patients who are no longer able to get to our office for care, we bring care to them. Physician-Nurse Practitioner teams visit patients in their homes, keep in contact with patients' families, and help coordinate patients' care with other medical and social service providers.

The goal of our Community-Based Program is to provide patients access to the full continuum of compassionate consistent medical care through their primary care team. In reaching this goal, we hope to preserve patients' independence and avoid institutionalization.

Nursing Home/Rehabilitation Practice

Since medical needs of individual patients change over time, our physicians and nurse practitioners provide medical services across a broad spectrum of settings so that patients have access to continuous, quality care throughout their lives.

In our long-term care practice, we deliver intensive medical care to patients who are chronically ill, residing in nursing facilities, and/or receiving hospice care. We also provide care on site at rehabilitation facilities for patients who need short-term services to improve function, typically after a hospital stay.
